Cyproterone acetate (CPA) is known to induce meningioma, and recently, nomegestrol acetate (NMA) and chlormadinone acetate (CMA) were also involved. Progestagen-induced meningioma management starts with progestogen discontinuation and is either interventional (surgery and/or radiotherapy) or conservative (clinical and MRI radiological follow-up). We performed a retrospective volumetric radiological outcomSe study of progestogen-induced meningiomas diagnosed in our hospital. We analysed progestogen-related meningiomas diagnosed until 30 June 2021, with at least one diagnostic and one follow-up MRI results. Meningioma volumes were centrally retrospectively measured using a T1-weighted 3D millimeter sequence with gadolinium injection on a postprocessing console. We analysed 98 meningiomas of 38 females and one transgender (male-to-female), of which 25 (64.1%) had taken CPA, seven (17.9%) NMA, three (7.7%) CMA, and four (10.2%) several progestogens. Eleven patients (24 meningiomas) underwent interventional management, seven patients had meningiomas followed by conservative or interventional management, and 21 patients (51 meningiomas) had only conservative management. Of these 21 patients, 17 had discontinued their progestogen less than 6 months before, of which 14 (82.3%) had decreased or stable meningioma(s) during a 24-month median follow-up (3 to 75) period. Overall, four of the 39 patients experienced meningioma progression (three during conservative treatment and one after surgery), including two patients who had continued NMA or CMA treatment several years after diagnosis. Our study confirms a generally favourable outcome of progestogen-related meningioma after conservative treatment, especially for CPA. It also underlines the need for progestogen discontinuation at meningioma diagnosis.

This comparative analysis evaluated endocrine profiles and gestation length data of captive pregnant black rhinoceros (Diceros bicornis), white rhinoceros (Ceratotherium simum), and greater one-horned (GOH) rhinoceros (Rhinoceros unicornis). Hormone profiles were collected over three decades as part of pregnancy diagnoses. After the third month of gestation, the luteo-placental shift in progesterone production in pregnant rhinoceroses causes a significant increase in the concentration of faecal progesterone metabolites. We defined a laboratory-specific value of 1000 ng/g faeces as a threshold for incipient feto-placental progesterone production. Using this value allowed a comparison between species and revealed significant individual differences within a species. The mean ± SEM gestation days for reaching the 1000 ng/g faeces threshold were 89.5 ± 2.9 (range 56-138 days; n = 39) in black, 96.0 ± 2.6 (58-138; n = 39) in white, and 117.8 ± 5.3 (74-173; n = 19) in GOH rhinoceroses. For the calculations of gestation length, we complemented our results from three decades of reproductive monitoring with data from the literature, resulting in about 70 values for each species. Gestation length in the black, the white and the GOH rhinoceros was 460.6 ± 1.5 (range: 436 - 486), 503.8 ± 1.3 (range: 480 - 525) and 480.5 ± 1.1 (range: 453 - 505) days, respectively. Daylight length significantly affected gestation length, while the sex of offspring had no effect. On average, pregnancies with parturitions in spring and summer were one week shorter than those in autumn and winter. Although rhinoceroses are non-seasonal breeders, most parturitions in captivity occur in autumn and winter. We also analysed preconception endocrine profiles in the white rhinoceros. Conceptions in this species occurred after oestrous cycles of approximately 35 days (n = 18), 70 days (n = 3), 15 days (n = 1), after periods of ovarian inactivity (n = 5), and during a foal heat within one month after stillbirth parturition (n = 1). In conclusion, this study provides a comprehensive overview of gestational parameters in three rhinoceros species.

This study describes patterns of steroid hormone production and activity for banteng (Bos javanicus), an endangered member of the Bovidae family. Using validated assays, concentrations of fecal progestagens, androgens, and glucocorticoids were quantified for four females and one male at the Saint Louis Zoo. A commercial activity monitor was also validated for assessing movement. The devices were then used to characterize activity in relation to season, reproductive status, and fecal steroid concentrations. General linear mixed models assessed differences in activity and steroid concentrations among individuals, in regards to reproductive status and season. Ovulatory cycle patterns, changes in activity around estrus and parturition, and events correlated with increased glucocorticoid production were also documented. Cycle lengths were 24.7 ± 0.4 days, and cycle lengths varied among individuals. Females cycled year-round, but luteal progestagen concentrations, along with glucocorticoids and male androgens, increased during the summer. Activity also increased in the summer. Progestagen concentrations were greater in pregnant females, and the gestation length of one pregnancy was 254 days. Pregnant females were less active overall, but activity increased the day before parturition. Activity was also greater preceding the onset of the luteal phase. The majority of glucocorticoid concentrations were in the range of baseline concentrations. However, a small number of elevated concentrations were correlated with husbandry and veterinary events. This study is the first to validate non-invasive methods for monitoring reproduction, welfare, and activity of banteng. Our results may contribute to the improved management of captive populations.

Background: Evidence on a possible association between newer hormonal contraceptives (HC) and risk of breast cancer remains inconclusive, especially as concerns progestogen-only methods. Methods: In this nationwide prospective cohort study, all Swedish women aged 15-34 at study start on January 1st 2005, or who turned 15 years during the study period, were followed until December 31st 2017. Using information from seven National Registers, we assessed the risk ratio of developing breast cancer and breast cancer in situ in relation to different HC using Poisson regression. We adjusted the analyses for several known confounders of breast cancer. Findings: This cohort included 1.5 million women providing more than 14 million person-years. During the study period, 3842 women were diagnosed with breast cancer. Compared with never users of any HC, we found no increased risk of developing breast cancer among current users of any combined HC, IRR 1.03 (0.91-1.16), whereas current users of progestogen-only methods had an increased risk of developing breast cancer, IRR 1.32 (1.20-1.45). Across all types of HC, the risk of developing breast cancer appeared to be highest the first five years of use (combined HC IRR 1.39 (1.14-1.69); progestogen-only methods IRR 1.74 (1.44-2.10). The risk disappeared ten years after the women stopped using HC. The absolute risk of breast cancer per 100,000 women-years was 22.4 for never users, 10.9 for current users of combined HC, and 29.8 for current users of progestogen-only methods. Interpretation: Current use of progestogen-only methods is associated with a small increased risk of developing breast cancer, whereas we could only detect an increased risk among users of combined HC during the first five years of use. This may partly be explained by a selective prescription of progestogen-only methods to women with risk factors for breast cancer, like smoking or obesity. As the absolute risk of breast cancer was small, the many health benefits associated with HC must also be taken into account in contraceptive counselling. Progesterone is a steroid hormone traditionally linked with female fertility and pregnancy. In current reproductive medicine, progesterone and its analogues play crucial roles. While the discovery of its effects has a long history, over recent decades, various novel actions of this interesting steroid have been documented, of which its neuro- and immunoprotective activities are the most widely discussed. Discoveries of the novel biological activities of progesterone have also driven research and development in the field of progesterone analogues used in human medicine. Progestogen treatment has traditionally and predominately been used in maintaining pregnancy, the prevention of preterm labor, various gynecological pathologies, and in lowering the negative effects of menopause. However, there are also various other medical fields where progesterone and its analogues could find application in the future. The aim of this work is to show the mechanisms of action of progesterone and its metabolites, the physiological and pharmacological actions of progesterone and its synthetic analogues in human medicine, as well as the impacts of its production and use on the environment.

Cheetahs have been the subject of reproductive study for over 35 years, yet steroid hormone activity remains poorly described after ovulation. Our objective was to examine and compare fecal progestagen (fPM), estrogen (fEM), and glucocorticoid (fGM) metabolite concentrations post-ovulation in pregnant and non-pregnant animals to better understand female physiology (1) during successful pregnancy, (2) surrounding frequent non-pregnant luteal phases, and (3) after artificial insemination (AI) to improve the low success rate. Secondarily, the authors also validated a urinary progestagen metabolite assay, allowing pregnancy detection with minimal sample collection. Fecal samples were collected from 12 females for ≥2 weeks prior to breeding/hormone injection (the PRE period) through 92 days post-breeding/injection. Samples were assessed for hormone concentrations using established enzyme immunoassays. Urine samples were collected for 13 weeks from 6 females after natural breeding or AI. There were no differences among groups in fGM, but in pregnant females, concentrations were higher (p < 0.01) in the last trimester than any other time. For pregnant females that gave birth to singletons, fGM was higher (p = 0.0205), but fEM tended to be lower (p = 0.0626) than those with multi-cub litters. Our results provide insight into the physiological events surrounding natural and artificially stimulated luteal activity in the cheetah.

High-quality follicles result in larger corpora lutea (CL), producing more progesterone, and having a fundamental role in pregnancy maintenance. For some sows, follicular growth takes place during lactation, and follicle selection occurs under a catabolic environment. As altrenogest inhibits follicular development, this study aimed to evaluate follicular growth, CL size, estrus expression, and subsequent reproductive performance of sows treated with altrenogest during the last seven days of a three-week lactation. A total of 81 primiparous and 319 multiparous sows were allocated to two treatments: CONT (control group) and ALT (20 mg of altrenogest/day during the last seven days of lactation). Subsamples of 20 primiparous sows and 97 multiparous were randomly selected to evaluate follicular growth and 26 multiparous sows for serum progesterone analysis at day 21 of gestation. On day 21 of pregnancy, CL measurement was performed by ultrasound. Once in estrus, sows were post-cervically inseminated with pooled semen doses with 1.5 × 109 sperm cells at estrus onset and every 24 h during the standing reflex period. Sows not showing estrus until 10 days after weaning were considered in anestrus. The variables weaning-to-estrus interval, CL size, litter size in the subsequent cycle, and piglet birth weight were evaluated using the GLIMMIX procedure and compared using the Tukey-Kramer test. Anestrus, pregnancy, farrowing, and adjusted farrowing rate were evaluated as binary responses using logistic regression. Follicular size was analyzed as a repeated measure during treatment and after weaning. Treatment was considered as a fixed effect. During the treatment period, follicular size was smaller in ALT sows than CONT sows (3.29 vs. 3.52 mm; P < 0.001). However, after treatment, ALT sows showed a larger follicular size than CONT sows (5.30 vs. 5.03 mm; P ≤ 0.01). There were less ALT sows showing estrus than CONT sows on days three (1.03 vs. 4.57%) and four (55.38 vs. 68.02%) after weaning (P ≤ 0.05), respectively. At 21 days after insemination, ALT sows showed larger CL size and lower CL size variation (P < 0.01) than CONT sows. Anestrus rate, pregnancy rate, farrowing rate, adjusted farrowing rate, litter size in the subsequent cycle, piglet birth weight, litter birth weight, and birth weight variation did not differ between treatments (P ≥ 0.14). In conclusion, altrenogest treatment during the last week of lactation concentrated estrus expression on day five after weaning, larger follicle and CL sizes; however, with no improvement in reproductive performance.

Decidual macrophages constitute 20-30% of the total leukocytes in the uterus of pregnant women, regulating the maternal immune tolerance and placenta development. Abnormal number or activities of decidual macrophages (dMs) are associated with fetal loss and pregnancy complications, such as preeclampsia. Monocytes differentiate into dMs in a decidua-specific microenvironment. Despite their important roles in pregnancy, the exact factors that regulate the differentiation into dMs remain unclear. Glycodelin-A (PAEP, hereafter referred to as GdA) is a glycoprotein that is abundantly present in the decidua, and plays an important role in fetomaternal defense and placental development. It modulates the differentiation and activity of several immune cell types residing in the decidua. In this study, we demonstrated that GdA induces the differentiation of human monocytes into dM-like phenotypes in terms of transcriptome, cell surface marker expression, secretome, and regulation of trophoblast and endothelial cell functions. We found that Sialic acid-binding Ig-like lectin 7 (Siglec-7) mediates the binding and biological actions of GdA in a sialic acid-dependent manner. We, therefore, suggest that GdA, induces the polarization of monocytes into dMs to regulate fetomaternal tolerance and placental development.

Reproduction is key to species survival, and reproductive physiology represents a high priority investigative area for conservation biology, as it provides a basic understanding of critical life-history traits, information that is helpful for the establishment of management strategies. Here, we generated knowledge about the reproductive endocrinology of the hoary fox (Lycalopex vetulus), a small canid (2.5-4 kg) endemic to open areas of the Brazilian Cerrado and listed in the Brazilian National List of Endangered species. Specifically, we utilized non-invasive hormone monitoring methods to assess oestrogen and progestagen metabolites from eight female hoary foxes housed in five zoological institutions in the state of São Paulo-Brazil. We observed the elevations of oestrogen and progestagen metabolites between July and September in six of the eight females. No significant evidence of ovarian activity was observed during other months. Two females, who shared the same enclosure, did not show a pattern of reproductive cyclicity. Based on these characteristics, we concluded that captive hoary foxes are seasonal monoestric, with the beginning of the oestrus cycle occurring mainly in July followed by 2 months of the luteal phase when conception does not occur. We suggest the dosage of faecal metabolites of estradiol and progesterone could be used to differentiate the reproductive period from a non-reproductive period in Lycalopex vetulus females, providing relevant information about their reproductive biology that may contribute to species conservation and management strategies, such as increased ex situ reproductive success.

Progesterone (P4) plays a key role in pregnancy establishment and maintenance; during early pregnancy, P4 stimulates the production and release of uterine secretions necessary for conceptus growth prior to implantation; therefore, exogenous P4 supplementation may improve embryo development. This study evaluated the effects of supplementation during early pregnancy with long-acting injectable progesterone or altrenogest on embryonic characteristics of sows and gilts. Thus, a total of 32 sows and 16 gilts were used. On day 6 of pregnancy sows and gilts were allocated to one of the following groups: non-supplemented; supplemented with 20 mg of altrenogest, orally, from days 6 to 12 of pregnancy; supplemented with 2.15 mg/kg of long-acting injectable progesterone on day 6 of pregnancy. Animals were killed on day 28 of pregnancy, and ovulation rate, embryo survival, embryo weight, crown-to-rump length, uterine glandular epithelium and endometrial vascularization were assessed. Treatments had no effect on pregnancy rate, embryo survival or endometrial vascular density (P > 0.05). Non-supplemented gilts presented larger and heavier embryos compared to gilts from supplemented groups (P < 0.05). Sows in the altrenogest group presented larger and heavier embryos compared to non-supplemented sows and sows supplemented with long-acting injectable progesterone. In conclusion, supplementation of sows and gilts with progestagen from day 6 of pregnancy can be used as a means to improve embryo survival without deleterious effects.

Combined oral contraceptive (COC) use is associated with small, albeit significant, increases in mental symptom scores, predominantly irritability, depressed mood, and anxiety. Yet, randomized prospective trials are needed to better characterize the women at risk for COC-induced negative mood change. Thus, the primary aim of this sub-study to a placebo-controlled randomized trial was to determine whether COC use influences emotional interference by negative and positive stimuli. Secondly, we wanted to evaluate what factors would predict depressive symptoms at the end of the trial, taking personality factors, history of mental disorders and other demographic factors into account. Sixty-nine women were included, randomized to three cycles of treatment with a COC (1.5 mg estradiol and 2.5 mg nomegestrolacetate) or placebo. An emotional verbal Stroop task was used to measure interference of emotional stimuli, in which participants were asked to only name the color of a presented word, while ignoring the meaning of the word. Four different word categories were used; neutral, positive, depression, and anxiety. For the second aim of the study, rating on the Montgomery-Åsberg Depression Rating Scale during the final days of the trial was used as outcome. We found no interaction between emotional verbal Stroop word category and treatment, indicating that COC treatment did not evoke any differences in emotional interference to the three word categories. Significant predictors for depressive symptoms at the end of the trial were trait anxiety at baseline and prior adverse mood effects by hormonal contraceptive use. Treatment (i.e. whether women had been treated with the COC or placebo) did not play a role in predicting depression scores at the end of the trial. In conclusion, we found no evidence that combined oral contraceptive use is associated with impaired cognitive-emotional processing. Instead, the main predictors of self-rated depression at the end of the trial were baseline trait anxiety and previous mental symptoms during hormonal contraceptive use.

El objetivo del presente trabajo fue evaluar el efecto del tratamiento con Altrenogest sobre la aparición de celo y el tamaño de la camada en cerdas primíparas. Setenta y dos hembras (Landrace x Yorkshire) con un peso promedio de 155 ± 5 kg se asignaron aleatoriamente en dos grupos: GT = grupo tratamiento (n=36), al cual se le aplicó un protocolo de sincronización de celo con Altrenogest durante 18 días (20 mg/día) sobre el alimento y GC = grupo control (n=36). Se registró la concentración de los celos del grupo tratamiento para medir la eficacia del progestágeno. El número de lechones nacidos vivos (LNV), muertos (LNM) y totales (LNT) fue registrado en ambos grupos para su comparación a través de un ANOVA, usando el software estadístico INFOSTAT. No se encontraron diferencias significativas para el número de LNM y LNT, mientras que el número de LNV se vio favorecido por el tratamiento (p<0.05). El 91% de los celos se concentraron entre el cuarto y quinto día de finalizado la administración del progestágeno. Podemos concluir que el tratamiento con Altrenogest en cerdas primíparas, mejoró el número de LNV a la vez que logra concentrar eficazmente la presentación de celos entre los días 4 y 6 post-tratamiento.


The objective of this study was to evaluate the effect of Altrenogest administration on estrus manifestation and litter size of primiparous sows. Seventy-two primiparous sows (Landrace x Yorkshire) with a weight of 155 ± 5 kg were randomly assigned to two groups: 1) GT or treatment group (n=36) that received Altrenogest administration during 18 days (20 mg./day) in the food and 2) GC or control group (n=36). Estrus manifestation was recorded as well as number of piglets born alive, stillborn and litter size. ANOVA was used for mean comparisons using INFOSTAT statistical software. No differences between treatments were observed for number of stillborn and litter size whereas number of piglets born alive was increased by Altrenogest administration (p<0.05). Estrus manifestation was observed between fourth and sixth day following Altrenogest treatment. We can conclude that the treatment of Altrenogest in primiparous sows improved the number of piglets born alive and synchronized estrus.

Este estudo avaliou a eficiência de três protocolos de indução de estro síncronizado em ovelhas da raça Santa Inês. Vinte e quatro ovelhas adultas foram equitativamente distribuídas em três grupos, de acordo com ordem de parto, peso corporal (kg) e escore da condição corporal. As ovelhas receberam implante vaginal de progesterona natural por seis dias mais 37,5µg de d-cloprostenol laterovulvar e 300UI de eCG i.m., 24 horas antes da remoção do dispositivo. Ovelhas controle receberam CIDR330mg de progesterona, e as demais receberam dispositivo absorvente intravaginal humano, tamanho mini, embebido com 200 (OB200) ou 400mg (OB400) de progesterona. Coletas de sangue foram feitas nos momentos D0 (antes da inserção dos dispositivos), D0+6h e diariamente, até um dia após retirada do dispositivo (D7). A progesterona (ng/mL) foi semelhante (P>0,05) em todos os tratamentos ao longo do período experimental, exceto no dia da remoção do dispositivo, quando as ovelhas controle (2,5±0,3) tiveram progesterona superior (P<0,05) às ovelhas OB200 (0,6±0,1) e OB400 (1,2±0,4). O percentual de animais em estro (100,0, 62,5 e 100,0%) foi semelhante (P>0,05) e o intervalo para o estro (46,3±3,9a, 26,4±4,5b e 31,2±5,8a,b) foi diferente (P<0,05) entre os animais dos grupos controle, OB200 e OB400, respectivamente. A taxa de concepção das ovelhas foi de 50,0% (4/8) para cada grupo (P>0,05). As the natural habitat of more and more species becomes depleted, captive breeding programmes have become established to bring species back from the brink of extinction. Monitoring the reproductive status of an individual is essential in order to improve breeding success. Traditional methods have involved stressful blood sampling, and thus noninvasive methods have been proven to be reliable alternatives for monitoring reproductive function in both captive and free-ranging animals. Subsequently, noninvasive methods have become an invaluable tool in longitudinal studies and conservation efforts, as animals can be observed without, or minimal human contact. The Beira antelope is a small antelope endemic to the northern part of the Horn of Africa. Population numbers of the Beira have been declining over the last few decades due to habitat fragmentation. We show here that the reproductive cycle of female Beira antelopes can be monitored noninvasively, by using faecal samples to analyse oestrogen (fEM) as well as progestagen (fPM) metabolites. The profiles of fPM and fEM of both females showed regular cyclic patterns in which the follicular and luteal phases could be distinguished. The overall mean cycle length is 22 days (range: 21-25 days), with a mean length of the follicular phase of 6 days (range: 4-7 days) and a mean length for the luteal phase being 15 days (range: 12-16 days). The suitability of these noninvasive techniques should assist in optimizing breeding efforts of this endemic small antelope in captivity. Being noninvasive, this method could also be a useful tool for monitoring reproductive function in the dwindling wild populations.

Due to the changes that took place since the 1970s, women have achieved important socioeconomic positions. Many tasks, including domestic and familiar ones, continue to be under women's responsibility, which leads to an overload work. Additionally, the female organism has its peculiarities due to hormonal changes. Adding all these factors up, women seem to be more vulnerable to stressing factors and consequently, might be prone to present several health problems. Within this scenario, one can point out insomnia as a highly prevalent disease among women, directly affecting performance and quality of life. Progesterone has an important effect over sleep, acting both as a hypnogenic and as a respiratory stimulant. Hormonal contraceptives are largely recognized among the modern society women; however, little is known about the effects of these drugs on sleep. This proposal hypothesizes that the use of hormonal contraceptives, mainly those based on progestagens could be a new therapeutic element for the treatment of insomnia and one more tool to be used to improve women´s sleep pattern and quality of life.

The ability to track gonadal function is facilitated by the use of endocrine and ultrasound techniques, both of which are important tools for optimizing reproduction and ensuring sustainability of fragile populations. With so many species now endangered, captive breeding is increasingly viewed as a means to sustain important insurance populations. As reproduction is key to species survival, understanding how to control and monitor ovarian function is vital. Through decades of study, we now have a greater understanding of the diversity, and plasticity, of reproductive mechanisms across taxa. Even within related species, there are marked differences in seasonal, environmental and social influences on ovarian cycle dynamics, ovulatory mechanisms, and responses to assisted reproductive/ovulation induction protocols. For most wildlife species, endocrine function is assessed noninvasively through analyses of hormones or their metabolites excreted in urine or feces. Perhaps it should not be surprising then, that major differences in metabolism and routes of excretion exist, not only between species, but also among hormone types within a species. This means that a species by species, and sometimes hormone by hormone, approach is essential for developing effective reproductive monitoring and control strategies. Over the past 30 years, our laboratory has developed and validated a number of reproductive assay techniques, which has led to our amassing a database of ovarian cycle dynamics on over 100 species. This paper presents an overview of ovarian physiology, and summarizes comparative ovarian function research on some of our most well-studied species: felids, elephants, rhinos, tapirs and the giant panda, and how that information has been used to aid ex situ management. Each of these species represents a range of reproductive strategies, from the highly seasonal, monestrus giant panda to the aseasonal, polyestrus elephant. Some species exhibit spontaneous ovulations, while others are induced ovulators or both, with variations in ovarian cycle lengths that range from a few days to several months. These differences reinforce the need for studies of species basic biology to optimize breeding strategies.

ABSTRACT: Gestation length in swine has a considerable amplitude and both early and delayed parturition are common. This variation increases the occurrence of unassisted farrowing and could lead to a wide-ranging age at weaning for piglets born from one batch. Supervision of sow parturition is crucial to reduce mortality of neonate piglets. To facilitate assistance, induction of farrowing using prostaglandin F2α (PGF) has been widely used in batch farrowing systems, whereby synchronization would concentrate the time of farrowing, allowing for better organization of employees. However, a viable alternative method that can be implemented to manage farrowing is to sustain high progestagen levels in the final days of gestation and, consequently, prevent early parturition. Efficient techniques to delay farrowing such as using oral progestagen supplementation have been previously described, but are only recently being considered for commercial use. The present manuscript reviews publications regarding delaying parturition and discusses the use of intravaginal devices (IVDs) containing progestagen. There is limited data addressing the effect of progestagen treatment during gestation on productive and reproductive performance. Therefore, future studies should focus on improving synchronization protocols following progestagen supplementation and evaluating piglet viability and sow fertility, before widely using progestagen supplementation to manipulate parturition.


RESUMO: Como a duração da gestação de suínos pode ter ampla variação, é comum a ocorrência de partos antecipados ou gestações prolongadas. Isso aumenta as chances de partos sem assistência e leva a uma grande variação de idade dos leitões dentro do lote de produção. Portanto, a supervisão do parto é indispensável para reduzir as perdas neonatais. Para facilitar o auxílio aos leitões, a indução do parto com prostaglandina F2α (PGF) é eficaz e amplamente utilizada, sendo indicada para concentrar os partos em momentos mais adequados, preferencialmente durante o horário com maior disponibilidade de colaboradores. Uma alternativa viável é manipular o momento do parto, através da manutenção de níveis plasmáticos elevados de progestágeno durante o final da gestação, a fim de evitar partos antecipados. Formas eficientes de evitar o parto através de suplementação oral de progestágenos foram descritas há décadas, mas apenas recentemente tem sido cogitada a utilização comercial. A presente revisão aborda estudos disponíveis na literatura relacionados ao protelamento do parto, incluindo a utilização de dispositivos intravaginais (DIVs) impregnados com progestágeno. São poucos os dados disponíveis relacionados ao uso de progestágenos na gestação com índices produtivos e reprodutivos. Portanto, alguns pontos ainda devem ser melhor avaliados, especialmente com relação à determinação da sincronia dos partos após o fim da suplementação com progestágenos, à viabilidade dos neonatos e à fertilidade subsequente das fêmeas antes da ampla adoção desta técnica.

OBJECTIVE: Ever since the introduction of combined oral contraception (COC), one of the major reasons for discontinuing the pill use has been mood-related side effects. Moreover, women who discontinue the pill turn to less effective methods whereby the probability of an unintended conception increases. Approximately 4-10% of COC users complain of depressed mood, irritability or increased anxiety, but drug-related causality has been difficult to prove. Given the lack of randomized controlled trials in this area, we aimed to prospectively estimate the severity of adverse mood in COC users that would be as representative of general users as possible. METHODS: This investigator-initiated, multi-center, randomized, double-blinded, placebo-controlled study included 202 healthy women. Women were randomized to a COC (1.5mg estradiol and 2.5mg nomegestrolacetate) or placebo for three treatment cycles. Main outcome measure was the Daily Record of Severity of Problems (DRSP), which was filled out daily during one baseline cycle and the final treatment cycle. RESULTS: Results from 84 women in the COC group and 94 women in the placebo group were analysed. COC use was associated with small, but statistically significant, increases in mean anxiety (0.22; 95% CI: 0.07-0.37, p=0.003), irritability (0.23; 95% CI: 0.07-0.38, p=0.012), and mood swings scores (0.15; 95% CI: 0.00-0.31, p=0.047) during the intermenstrual phase, but a significant premenstrual improvement in depression (-0.33; 95% CI: -0.62 to -0.05, p=0.049). Secondary analyses showed that women with previous adverse hormonal contraceptive experience reported significantly greater mood worsening in the intermenstrual phase in comparison with healthy women, p<0.05. The proportion of women who reported a clinically relevant mood deterioration did not differ between those allocated to COC (24.1%) or placebo (17.0%), p=0.262. CONCLUSION: COC use is associated with small but statistically significant mood side effects in the intermenstrual phase. These findings are driven by a subgroup of women who clearly suffer from COC-related side effects. However, positive mood effects are noted in the premenstrual phase and the proportion of women with clinically relevant mood worsening did not differ between treatment groups.

Summary Background: it is known that coffee pulp can modify the oxidative status and fertility in dairy cows. Objective: to evaluate the effect of dietary supplementation with coffee pulp on the antioxidant capacity, lipid oxidation and reproductive characteristics of ewes during estrous synchronization and early gestation. Methods: forty Dorset-Suffolk crossbred ewes with 3 or 4 parturitions were allocated to two treatments: T0 (n = 21), ewes supplemented with 450 g of a control feed; and T1 (n = 19), ewes supplemented with 450 g of the feed with 25% coffee pulp. Supplementation began 14 days before estrous synchronization and ended 25 days after breeding. During estrous synchronization, progestogen (CIDR, Controlled Internal Drug Release) was inserted and left in situ for 11 days. Eighteen hours later, estrous detection began with the aid of rams. Blood samples were collected at different times of synchronization and during early pregnancy to determine antioxidant capacity, lipid oxidation and blood progesterone concentration. Pregnancy diagnosis was performed 30 and 60 days after CIDR removal. Results: supplementation with coffee pulp did not affect estrous onset, estrous response or progesterone concentration, but fertility decreased from 100 to 78.95%. The antioxidant capacity measured using the FRAP technique was greater in coffee pulp supplemented ewes only before progestogen insertion. Coffee pulp did not modify lipid oxidation; however, this variable was affected by sampling time, decreasing after progestogen removal to its lowest values at 22 days into pregnancy. Conclusion: although supplementation with coffee pulp at 25% in the concentrate increased antioxidant capacity of ewes before insertion of progestogen, it is not recommended to use this percentage during synchronization or early pregnancy since it can negatively affect gestation rate.


Resumen Antecedentes: se sabe que la pulpa de café puede modificar el estado oxidativo y la fertilidad en vacas lecheras. Objetivo: evaluar los efectos del suministro dietario de pulpa de café y su capacidad antioxidante, oxidación lipídica, y características reproductivas en ovejas durante la sincronización del estro y la gestación temprana. Métodos: cuarenta ovejas cruzadas Suffolk x Dorset de 3 y 4 partos fueron asignadas a dos grupos: T0 (n = 21), suplementación con 450 g de alimento (grupo testigo); y T1 (n = 19), suplementación con 450 g de alimento con 25% de pulpa de café. La suplementación inició 14 días antes de la sincronización del estro y terminó 25 días después del apareamiento. El progestágeno (CIDR, Dispositivo Intravaginal de Liberación Controlada) fue insertado en los animales por 11 días. Dieciocho horas después de su retiro se inició la detección de estros. Se muestreó a diferentes tiempos después de la sincronización y durante la gestación temprana para determinar capacidad antioxidante, oxidación lipídica y concentración de progesterona. El diagnóstico de preñez se realizó 30 y 60 días después de la remoción del CIDR. Resultados: la suplementación con pulpa de café no afectó el inicio del estro, la respuesta al estro ni la concentración de progesterona. Sin embargo, la fertilidad disminuyó de 100 a 78,95%. La capacidad antioxidante, medida mediante la técnica FRAP, fue mayor en ovejas suplementadas con pulpa de café, pero solo antes de la inserción del progestágeno. La pulpa de café no modificó la oxidación lipídica; sin embargo, si fue modificada por el tiempo de muestreo, decreciendo desde la remoción del progestágeno hasta los 22 días de preñez. Conclusión: aunque la suplementación del concentrado con 25% de pulpa de café incrementó la capacidad antioxidante antes de la inserción del progestágeno, no se recomienda ese porcentaje durante la sincronización y gestación temprana, ya que redujo el porcentaje de gestación de las ovejas.

Objetivo: avaliar a polpa de café na capacidade antioxidante, oxidação da gordura e nas características reprodutivas das ovelhas durante sincronização do estro e gestação inicial. Métodos: quarenta ovelhas cruzas Suffolk e Dorset de 3 e 4 nascimentos foram agrupadas no T0 (n = 21), suplementação com 450 g de alimento controle e T1 (n = 19), suplementação com 450 g de alimento com 25% de polpa de café. A suplementação iniciou 14 dias antes da sincronização do estro e terminou 25 dias depois do acasalamento. O hormônio progestina (CIDR, dispositivo intravaginal de libertação controlada de fármaco) foi inserido por 11 dias. Dezoito horas depois da retirada iniciouse a detecção do estro. Fizeram-se amostras de diferentes tempos do período e da gestação inicial para determinar a capacidade antioxidante, oxidação dos lipídeos e concentração de progesterona. Realizou-se o diagnóstico de gestação 30 e 60 dias depois de remover o CIDR. Resultados: a suplementação com a polpa de café não afetou o início do estro, a resposta ao estro e a concentração de progesterona, mas a fertilidade decresceu de 100 a 78,95%. A capacidade antioxidante que foi medida pela técnica de FRAP foi maior nas ovelhas suplementadas com a polpa de café somente antes da inserção do progestágeno. A polpa de café não modificou a oxidação dos lipídeos; no entanto, estes foram modificados pelo tempo de amostra, decrescendo depois de remover o progestágeno até 22 dias de gestação. Conclusão: ainda que a polpa de café a 25% de concentração incrementa a capacidade antioxidante antes da inserção do progestágeno, não é recomendado este percentual para as ovelhas durante a sincronização do estro e a gestação inicial, já que decresce a porcentagem de gestação.
